1. loka = kāma-loka + rūpa-loka + arūpa-loka

'world', denotes the 3 spheres of existence comprising the whole universe, i.e. (1) the sensuous world (kāma-loka), or the world of the 5 senses; (2) the fine-material world (rūpa-loka), corresponding to the 4 fine-material absorptions (s. jhāna 1-4); (3) the immaterial world (arūpa-loka), corresponding to the 4 immaterial absorptions (s. jhāna, 5-8).

The Blessed One said: "And what is the origination of the world? Dependent on the eye & forms there arises eye-consciousness. The meeting of the three is contact. From contact as a requisite condition comes feeling. From feeling as a requisite condition comes craving. From craving as a requisite condition comes clinging/sustenance. From clinging/sustenance as a requisite condition comes becoming. From becoming as a requisite condition comes birth. From birth as a requisite condition, then aging & death, sorrow, lamentation, pain, distress, & despair come into play. This is the origination of the world.

The sutta quoted above says that these six thoughts of the thought-system create the loka. For your reference, I said already in the posting of 'thought-system is built by nibbana through paṭiccasamuppāda' that the thought-system is an aggregation of six thoughts while a thought is an assembly of citta mano and vinnana.

4. thought equals kamma

The sub titles of this writing, '2. kamma creates loka.' and '3. thought creates loka.', these two obviously lead to 'thought equals kamma.'
